as might be brought about by a phosphodiesterase inhibitor是一个插入语，as作代词指代elevated concentrations（其实应该根据逻辑意思来判断，也有可能指代periods，要看回原文！ ）。全句结构：

prolonged periods could lead to 。。。and to。。。

偶的翻译较烂，就不献丑了！

a not unusual occurrence with psychoactive drugs是effects的同位语，一种并非不寻常的并发症。

这是语法问题啊，我也搞不太清楚了。建议你将自己的问题到语法区问问吧。
