I don't think so, just to be little bit more informal should be enough.

at the beginning of the essay, do I need to address him? like "How are you Dr...., my name is Michelle...."?

No, don't need those greetings. Just get to the point directly. Also, even in the elevator, you still need to present yourself at professional manner.
