These are exceptions and so rarely happen at all. With a publication or R & R paper at a top journal, one tends to have high expectations - often unrealistically high such that eventually she ends up not finding what she wants on the job market. Also, exceptional candidates have a far greater variety of career options (this is especially true of the economics filed), including those in industry and research institutions. In addition, there are other conceivable reasons, such as insufficient language skills.
By the way, candidates from economics PhD programs that rank below the top 30 can hardly find faculty positions. This is true even if they have decent publications.
