relate any experience you can point to that indicate the applicant's level of creativity, motivation and initiative.

下面留了2行。这句话是不是和in what capacity you know the applicant一个意思，只要说明推荐人和我的关系就可以？

但是只留了2行地方呀，也就能写一句话的。而且后面有做s'ta'tement的地方。

you can use an additional sheet and ask your recommender to cover all of the questions in that seperate paper..
