tuck的financial application?

提交了online application 后，发现financial application(scholarship/loan)需要单独申请，请过来人指教一下好吗？

according to the instruction, financial application includes 2 items: 1) financial application form 2) copy of tax form in the past year

Is there anything else need to submit? Mail or Email?
