推荐信和成绩单的request form

是否一定要和成绩单/推荐信一起寄回呢？我们学校教务处才不会有那个耐心给学生填Request Form 呢。如果不填有没有关系呢？

I think it's not necessary to fill in that form.

I only heard about the request form for transcripts, which is not important. so you can just ignore it.

but the request form of reference, never heard about that. if you mean the evaluation form, you can ask your recommender to fill it and mail back along with your recommendation letter.(remember treat it like the recommendation letter)or you can just ask your recommender to write a general letter for you and ignore the request form.
