Hmm.. if you're really worried, try GMAT Prep and see how you do there?
No one, myself included, knows why we make careless mistakes (and I make a lot of them) so you won't gain much from dwelling on that. Try varying the time you spend on the first 10 questions, don't rush, be very careful and double check your answers, figure out what will help with reducing your error rate.
