The admission committee takes no more than a cursory glance at an applicant's application form, usually as the item placed on top of his/her package. That said, in case the disability noted therein catches their eyes, that will almost surely adversely affect his/her chances, especially if the candidate pool is particularly strong overall. Remember, the idea of working with a student with a disability generally does not appeal to a faculty member.

My take is that if the condition is minor or whether it is considered a disability is country/culture - specific, you do not have to include such information. Otherwise, in order to be on the safe side only apply to those where reporting disabilities is optional.

I also see many universities claim that they would always hire at least 7% of individuals with disability. I'm wondering if there is an invisible quota for people with disability in job application like pre-doc or faculty members hiring. Sometimes I hear you can contribute to diversity when disclosing your disability.

Disregard those pieces of misleading, nonsensical information. If you disclose a disability on your application form, and it is noted by the committee, you will almost surely be discriminated against.

You said it, raccoon-paw! Do not disclose anything if you want your Ph.D. application to be successful.
