Transcripts — Sealed envelopes containing Transcript Request Forms and official
transcripts or grade sheets from all colleges and universities attended.
The Admissions Committee requires an official transcript from each of the colleges and universities you have attended as a registered student (regardless of the number of courses taken), including summer school, evening classes, etc. Send the Transcript Request Form along with a self-addressed envelope to the Registrar of your institution(s). The Registrar should enclose both an official transcript and the Transcript Request Form, seal the envelope, sign across the seal, and return the envelope to you. When you receive the returned envelope, Do Not Open It. The sealed envelope must be enclosed with your application. If your institution(s) will not complete the transcript request form, you may submit your transcript without the form.
If you have attended more than one school, please make copies of the Transcript Request Form and ask the Registrar to sign across the seal of his/her institution’s standard business envelope.
If you studied abroad, and the courses have been transferred back to your institution and appear with grades on your transcript, you need not request a transcript from the foreign institution.
International Students: If you were educated in a country where your original transcripts are not in English, you must send both an official transcript in the original language and a certified English translation along with proof of graduation.
