个人见解：
D和E在although引导的分居中，如果用the rate做主语，后面的分句比较的对象就不明确了（france到底跟哪个国家比？），因为这两个分句都是相对比较独立的了，虽然主语都是rate，但是rate的内容不一样，也不能想当然的把前一个分句rate的两个比较项当成是后一个分句的
而B，although分句中people做主语，后一个分句可以理解为the death rates （of people in France and the United States），gamt中貌似有这样的省略规律，这样的话整个句意更加清晰、明白

其实我一开始选的E。。。。。

还不知道对不对，先谈谈自己的思路。OG里面一直强调说平行结构要工整，即使不是平行结构，工整的表达不容易引起歧义。所以应该是rate和rate之间的比较。CD说的是fatty food的rate，改变了原意，AB中although所带的从句的主语是人，不是rate，不够工整。所以选E.
如果不对的话还请各位指正啊~

有一个疑惑，如果their前面有两个复数名词，但从逻辑上将只能修饰其中一个，那么这是算不算指代不清呢？

这道题选B阿 E我认为错在the rate of people consuming fatty foods
consuming fatty foods 作为现在分词修饰people 这个短语的意思是在不停消耗食物的人的率
显然错误
同理D也是这个错误
