How to request GMAT scores after two years

 Hi all! So happy to be back again after two years ... lots of happening during the two years for me, I'd like to share with you my nyc/london school/work experience later.  However here I'm back, with my new dreams :-)

It's shameful that I forgot  my account name here in CD (gai da) and forgot GMAT scores and dates taken two years ago in China ... any body can help with advising on What's the feasible way to ask ETS for the scores and request delivery with my name and passport number (gosh, did we register with passport number??)
