WES的申请确认表找不到了，求问！

WES我已经付过费了，但是没有打印这个表格，再登录就找不到了。
搜索了一下以前的帖子，说是登陆后会显示一个Here is a copy of your completed application for your records
可是我登录进去没有啊，只有
Our system indicates that you have already submitted an application to WES.
If you would like to view the status of application or update information or service options, please go to My Account section to make any changes or adjustments.
只能进my account，里面可以看到申请，写着状态是“Waiting for required documents ”，根本没有能打印那个确认表的东西。
WES给我发过邮件，里面有application confirmation但是格式不一样，这个可以用吗？
另外还有一个 Request for Academic Records 这个要寄吗？
