有没有申过港中文MSC in finance 的同学，他们的材料到底是怎么寄的呀，求助！

(1)   This section should be completed by the applicant for requesting one copy of official transcript to be sent directly to the Admissions Office of the MSc in Finance Program.

(2)   Upon completion, the applicant should send the form to the Registrar of the University/College from which the transcript is requested.

这样说的意思，难道是要寄到两个地方去么。。。。。一个寄成绩单，一个寄表格。。。。而且话说地址是什么啊，完全没看到写。。。求解。。。。
