You ask some good questions, and let me try and help you out.
1. It's okay if your recommender doesn't speak English, as he can get the recommendation translated. When choosing your recommender, the most important thing is choosing the person who will give you the best, most excited recommendation
2. All schools pretty much ask very specific questions. Once you open your application and put in your recommender's email, they will get access to the recommendation which will ask very specific questions.
3. They will have to. These days as you might guess everything is electronic, and the recommenders will have to fill out a different recommendation for each school, even though the content will be similar.
