我觉得是从句子意思上判断的，应为意思是对牛奶的过敏反应，而不是牛奶，所以需要to

但我想：C选项，to identify food allergies, such as to 补充完整是to identify food allergies, such as to milk allergy
这样省略就比较自然了。

C的such as to，其实是such as the allergy to milk，红色部分省略掉，allergy to sth.是固定搭配。
——这是我认为合理的解释，不过我不太确定为啥可以这么省略？欢迎讨论

似乎比较ambiguous，给人的感觉是"to ..."的平行结构，"milk allergy"是"identify food allergies"的一个例子
to identify food allergies, such as the allergy to milk
这样就是把food allergies和"the allergy to milk"平行，后者是前者的特例
我还是觉得这个to不是为了呼应前面的不定式".... tests to ...."——否则，就该是to identify food allergies, such as to identify milk allergy
而是"allergies to sth"

只是加入了插入语成分, Manhattan上面说的，A short non-essential phrase intervenes and is set off by commas

我觉得，那个that修饰的是food allergies，而不是修饰such as to milk中的milk，因为such as to milk是作为插入语存在，不应该妨碍that修饰前面的food allergies.
从语法上考虑，用that指代，前面不应该有逗号。而且that作为题干，一定是正确的，所以能印证上述红色部分的正确性。
