1、双层并列。这种结构，我从来没有看过，所以我不敢确定是否是对的。

2、副词有时候可以修名词。

104. The diet of the ordinary Greek in classical times was largely vegetarian--vegetables, fresh cheese, oatmeal, and meal cakes, and meat rarely.

从句法上分析，破折号后的部分是对vegetarian进行解释的。而这个结构中，vegetables, fresh cheese, oatmeal, and meal cakes作为一个词组，and meat rarely./(E)  with meat as a rarity是对前面的词组进行补充说明的。所以用 with 这种结构。

另外，有个不懂的地方：

104. The diet of the ordinary Greek in classical times was largely vegetarian--vegetables, fresh cheese, oatmeal, and meal cakes, with meat as a rarity.

句中的逗号, 为何不省去？
