答案是A，但想问一下A中的 not only do damage by themselves but also aggravate中的do 是什么个用法？直接not only damage不行吗？

我认为不能直接用not only damage，因为but also后面是谓语+宾语的结构，前面的not only也应该是谓语+宾语结构。

damage是及物动词，后面应该要跟宾语。Longman上的例句可以看出，damage后面都跟宾语。所以not only damage是错误的，没有这种用法。

do damage里面的damage是名词，这是一种习惯用语。Longman里面的解释和例句是：
COLLOCATIONS
verbs
do damage    
例句：Too much sun can do severe damage to your skin.

所以可以把do damage联合起来看作谓语，后面如果要跟宾语可以加to.

综上，这里的do damage不是为了和后面平行，而是damage的习惯用法而已。
