for 1) e.g. the failure essay, Are you saying that i may use the same example but must say something different about what the failure brought me? Adcom will have no interest on the identical essay resubmitted this year, right?

for 2) it's really hard for my former employer to comment on my progress this past year as I have left the former work place several years ago. i don't know what he wrote last year, but I guess he will copy and paste his last year's letter if I ask for his letter this year again. after all, the examples were facts, how could he change it? Do you think this will be a severe disadvantage for me?

1) You can certainly use the same material, but I would suggest looking it over again to really make sure that there's no way you can improve the essay. Like I said, people frequently submit less perfect essays than they thought they did. Getting new people to take a look at the essays may be one way to get a fresh perspective.

2) It is in cases like this that you may want to think about adding the optional third recommendation. Did you do some other extra-curricular activity (eg volunteer work) in the past year, that you can back up with an optional recommendation letter? This would be a letter that 'backs up' the first two, but wouldn't typically contain enough substantial content for an applicant to submit it as one of the two required recommendation letters. Also I would use this only if it brings in a new perspective. If it merely brings in another perspective from your work, for instance (which your current supervisor should already cover), then I would stick with just two recommendations.

No, you don't have to have graduated from a 'brand-name' university. More important than 'brands' (and this includes work experience), is what you did in your experiences.

If you're living in New York studying at Columbia, firms in New York might expect that you will be at all their events because it is so easy for you to visit them. If you're in Chicago recruiting for New York positions, then even if you go once every two weeks or once a month, firms will think it's enough because they know you're further away.

The point is, some industries (eg. banking) are very 'high-touch', and really need you to show your interest by doing a lot of networking. But even then, they understand that for some it's geographically easier to network and so expect even more from you.
