我排除A选项是因为imposing。通过阅读全句，我们可知划线部分都是在说明insurance plans，所以这里用that＋从句比doing更准确的修饰plans。至于你说的“为什么不可以require doctors to see and (to) spend, 而要用现在分词spending呢“  我也不确定要用spending，只是我在C、D、E的比较中发现D最好（C中的and spend错，变成了plans spend；E中impose，requiring， and spending错，不对称），所以选出D相对最佳选项

PP61中的A选项and spend歧义，可以理解成和are less likely平行，这样主语就是members了 union members spend less time with each逻辑有错

首先按楼主的意思，这样省略会造成歧义，因为不知道spend和see平行还是require平行。另外，观察一下意思，要求医生花更多的时间来互相相处这个语义是不对的吧，应该是花更多的时间两者来相处，D表达了这个意思，spending这个结构是做结果状语的，因为ving结构的逻辑主语是整个句子，所以来做结果状语，严格控制medical services的结果是让双方有更多的时间相处，这个意思的正确的。
