急问一个填申请表中遇到的问题~~！

今天填申请表，填到这个选项时不知道该怎么填了，请教大家阿!~~

If you need visa eligibility documents from the University of Illinois, please indicate type	F-1                             J-1                             Need, but do not know type
If you do not need visa eligibility documents, indicate Program Sponsor that will be issuing them.

第一个下拉选框有以下选项：F-1，J-1，Need，but do not know type.

第二个是自己填写的

急问阿~~求教各位了！~~

另问申请表中的 Signature page  还有其他form的签字是print signature 敲上去阿还是 scan hand handwritten signature 扫描上去阿？？

还有 (Optional) I hereby waive whatever rights of access I might have to this confidential recommendation as provided in the Federal Family Educational Rights and Privacy Act of 1974. 需不需要选阿？

都是比较弱的问题不过很着急的问~~谢谢大家阿！

应该是选F－1.

签字基本都是敲进去的。没听说要scan后放进去的。

推荐信最好还是waive access啦。虽然有可能是你一手炮制的。

还有一个：In which areas could the applicant exhibit growth or improvement? 是要让推荐人描述申请人以后可以提升的地方还是已经提升的地方啊？？
