"Mail official transcripts from all undergraduate and graduate institutions you attended

“from.........institutions you attended"是transcripts的后置定语，意思是“（你所读过书的所有学校的）成绩单。” from后面带的是修饰transcripts的形容词成分，并非作为修饰mail的副词成分。

清楚否？

同意。如果要让学校寄，应该这么说：Have all undergraduate institutions mail your transcripts to ...

Yale requires all the transcripts to be mailed from institutes you attended, not from the applicants.  But I mailed them by myself.

你的是错误的。YALE是让你把学校成绩单拿来，和其他申请材料（比如推荐信，如果你有别的材料的话）装在一个信封里，一起寄给他们。
