参考答案选E，可是这个they不能由日本老人这个群体转意到西方老人这个群体上啊，如果用E的句式，我觉得应该是than those who are in most western countries.或者than in most western countries(than后省略old people).

Sth. in A is better than that/those in B.

这里that/those不能用it/they取代，因为A中的sth.和B中的sth.是两个群体阿。Right??
