1. it我觉得应该是指代名词或者词组的.

2. 如果选择whom的话, 那就是say的宾语, 那后面的has引导的谓宾结构就不知道和谁搭配了.

3. 省略的前提是清晰, 如果不一样的动词, 省略了别人就不知道说什么了, 至少逻辑判断会当成省略了和前面一样的那个动词.

谢谢楼上两位，后面两道OK了，但第一题仍有疑惑点，假如按likui说的指代what，那把what替代补全，则为not only by what patients eat but also by when they eat what，好象没这种用法吧，it我觉得应该是指代名词，而不是关系代词，但是到底指代什么，又搞不明白，继续请教
