If you wish to purchase a duplicate examinee score record, you must submit your request in the form of a letter.  lease be sure to include your name, date of birth, mailing address, name of test, test date, and registration or appointment confirmation number and enclose the required fee. The service fee is US$17. Scores are mailed within two weeks after receipt of your request and payment.
If you have a VISA, MasterCard, Discover, JCB, or American Express credit card, you may fax your request to 610-290-8972. Be sure to provide your credit card number and expiration date.
You may also telephone your request to 1-888-863-3544. (Outside the United States and Canada, call 1-609-771-7100).
