Congregations on your recommended admission for the MＡＣＣprogram at the ＸＸＸ .  To complete your visa paperwork we need a completed International Student Verification Form.  You may fill out the attached form and mail, f or e-mail the document to the MAＣＣ office.  Or, you can fill out the form online using the same web site and system that you used to apply for the MAＣＣ　program.  You only need to fill out one form and send it in.  It may take up to three week to process your application after we receive a completed form.
