B. rules with the intent of ensuring one nurse at least to be assigned for every four patients to be put through triage in a hospital emergency room.

黄色部分意思不对。

rules with the intent of ensuring 似乎是作为插入语,没有A中的动词表达形式好。

B,awkward;C,"put through triage" modifies "room"; D,  That-clause modifies "room"; E, "this" refers to ?. So,  the correct answer is A.

你说的是原题划线部分的句子成分吗?它作同位语,对修饰对象起解释说明的作用。

'is assigned' and 'put through' are 2 verb, how can they use together
