Can you tell us which school's application is this? I may be able to tell what it means once I see it in the context. But here is my best guess - "Initials" usually refers to the first letter of your first name and last name, which is a simplified version of your signiature. For example, if your name is John Smith, your initials is JS. Once you come to US, you will have plenty of oppertunities to write down your initials on all kinds of legal documentation. So, it seems to me the application needs you to type in your initials if you request a recommender waiver.
