这句话的层次是这样：1）any medical test will sometimes fail to...when... and indicate...when...，and 前后是对比的关系。换言之就是fail to detect a condition when it is present v.s. indicate the condition is present when it is not (present)(当这个症状出来的时候没发现v.s.预测有这个症状的时候实际上没有)

A答案and前没有问题，但是后面哪个one有问题，因为都说的是a condition, 是一个特定的condition, 而不是一个泛指的condition, 而one是不定代词，指的是一群东西中的某一个或者指一个不定的东西 Manhattan SC 5th p234
BD 都有一个问题，少了a condition, 原句的意思被改变，是detect a condition而不是强调发现这个condition的时间
E 冗余，原句意思也被改变，发现的是a condition而不是the presence of condition, there没有先行词
