怎么知道ONLY修饰的是 by heart disease and cancer，而不是surpassed呢？
我觉得修饰by heart disease and cancer和surpassed似乎是一样的

显然不一样，如果修饰的是surpassed, 说明diabetes surpassed heart disease and cancer
but， 如果修饰的是 heart disease and cancer，那么意思就完全不一样了，是heart disease and cancer only surpassed diabetes

要是only surpassed by sth. 那only修饰的就是surpass，就相当于强调【只有surpass】一个动作，暗含意味是【没有其他的动作了，只有surpass一个动作】。
但是如果变成surpass only by sth. 那only就修饰的是by sth. 强调的就是【只被XXX】超越。。

具体你可以类比：
He only kissed me.  意味着【他只是亲我了】，没有抱我，或者打我。。。。   = =
He kissed only me.  意味着【他亲了的人只是我】，意味着他没有亲别人。。。 = =

不论only在surpass前后，说的都是heart disease and cancer surpass diabetes。

这个我不明白，如果真的是修饰对象要紧跟，那么不也应该写成：
“surpassed by only heart disease and cancer ”么，才是你说的意思阿
