I do not have any doubt about the structure highlighted in red. Definitely "cigarette smoking" and "alcohol consumption" are strictly parallel in structure. That does not mean that the expression "eating rich foods high in fats" could not be part of the parallel, as it also belongs to the category of noun phrase, as do the other two.

Sometimes, we should not be too critical on the grammar.

"withdrawal", "reductions", "raising", "pardon"是做demand的直接宾语，当然有名词的话做好用名词。再看，如果是“demand raising"的形式正确，则说明"demand+doing"是固定搭配。这和习惯似乎不符，因为常规搭配是"demand+sth"

上一个句子，我想“as+doing”是可以接受的。
