UW 的 Financial Ability Form 求助

那啥。。。我想问问UW的Financial Ability Form...
有条这样写道 ：
4. If dependent on university financial support such as a research/teaching appointment: Contact the graduate program you are applying to for further information. Check （此处省略一对号）the fourth statement in section 2 of the UW Statement of Financial Ability Form, sign the bottom of this form, complete section 3, (declaring dependents) and return.
但是 第四条明明是

______I have submitted an application to be considered for a Teaching or Research Assistantship
from the department.

这有啥关系？？？又不是dependents。。。
大家都怎么填的哇~~
有人晓得不？
填过的没填过的同学们都来帮帮忙~~

PS：本人木有dependent，但是申了GA~~
