请问题目中的surpassed only…… 是做状语伴随结果 还是定语修饰Diabetes??? 请教了……如何区分呢？

不好意思 题目是
5.    Diabetes, together with its serious complications,
ranks as the nation's third leading cause of death,
surpassed only by heart disease and cancer.

(A)    ranks as the nation's third leading cause of
death, surpassed only

先声明，俺不是牛，俺刚开始做OG。仅供参考，希望大牛指导。

就楼主问题，俺觉得是伴随状语，逻辑主语是diabetes，修饰的是谓语动词ranks。如果被认为是定语，好像GMAT里面喜欢用-ed修饰就近的名词，也就是这里的death，另，如果是做定语修饰主句主语，貌似离得有点太远了吧。。

only在修饰by heart disease and cancer

如果变成only surpassed by就是修饰surpassed了。。

OG说修饰语要靠近其所修饰的东西

PS：我也不是N~~这只是我的看法而已。
