该句中在and后面省略了to。     be likely to do and to do.
that 从句修饰的是insurance plans。 spending表伴随状态是修饰其前面的整句话的，即“因为要求医生看更多的病人，那么这样一种状态导致了医生花在每个病人身上的时间就变少了”。

谢谢msqjz501，虽然spending表示了伴随结果状态，但是它的主语难道不应该是前面从句的主语insuarance plans吗？从意义上讲它的主语因该是doctors，但是doctors在前面的从句中显然是一个require的宾语，就是这一点让我很困惑。

我所查阅的所有资料，包括平时看到的句子，都表明“逗号＋现在分词”的结构，后者的逻辑主语就是逗号前其所修饰的句子的主语。
