为什么A选项是正确的，不是说如果句子中出现相同代词的话，代词指代应该一致么？但是A中第一个it 是形式主语，第二个it应该是the Washington Post.

我觉得两个都理解成强调句的成分比较好，强调句可以吧it is that 去掉还原
还原成正常句式是
it （第一个分句里第二个it）moved into the first rank of American newspapers only after Katharine Graham became publisher of The Washington Post in 1963，the paper won high praise under her command.

是说如果是强调句可还原的情况下，it 不作代词用么？那么强调句中的it 做句子的什么成分？

it 是代词，也是个形式主语，指代that 后面的所有东西（可以理解成一件事），你说的指代歧义在这种固定句式中应该是不存在的， 因为一个指代一个句子，一个指代一个名词
如果有两个或两个以上的it都是指代名词 ，那么这种用法是有歧义的，应该避免
