Diabetes, together with its serious complications, ranks as the nation's third leading cause of death, surpassed only by heart disease and cancer.
我有两个问题：
1、这个地方的ranks为什么不用成is ranked,我理解为应该是“被列为”？
2、OG中对“together with its serious complications”的解释是：the phrase following diabetes is set off by a pair of commas, indicating that it is off by a pair of commas, indicating that it is descriptive information that may be dropped from the sentence, it is not a part of the subject.以后我怎样可以判断它属于，还是不属于主语？

我认为surpassed 放在by 后面，修饰的重点在于B&B这两个疾病上，强调A比仅仅比他们的死亡率高
而only surpassed 重点则在surpassed这里，是说surpassed这件事情，改变了句子的重点
我记得有XDF的老师说过only的位置不要改变，当然不绝对，有特例，但是大部分题可以这样处理的

OG上说only放在by前面是限制后面的词组
A surpassed only by B&C 指的是A仅仅被B&C超过

如果是only surpassed，限制不明确，求指正。

By the way，我留言的意图是，我觉得有一个地方我和你的观点不同，
即使去掉逗号，变成了Diabetes together with its serious complications，它的谓语动词仍然是单数，因为together with 要随前一致，则依照这道题，谓语动词仍然要和diabetes一致还是使用单数。
