求助，关于毕业证错误的更正说明！！

毕业证上将我的生日写错了，想写一个更正说明（英文）让学校盖章，不知该如何写!  求助大家

We hereby make this official identification for Miss ***, former undergraduate enrolled in 19** as full-time with major of *** in Department of *** in School/College of ***, that her date of birth issued in our official Certificate of Graduation has now been proved inaccurate. 这种事情办起来肯定很难，现在关键是要找出当时你毕业证上出生日期错误的原因，是因为你当时提供错了还是当时行政人员疏忽？如果是你自己提供错了，这事就麻烦了。
