spending处于句末一般修饰前面的句子，不过这里修饰句子不是太通顺，理解成修饰最近的主谓动作也是可以的～

impose和require的主语是insurance plans
而spend的主语是doctor
spending less time with each是修饰doctor的。而不是与impose和require一个层面的平行
