条件状语从句中，会有从句一般现在式，主句一般将来式。
但本题中从句过去式，主句一般现在式？如何解释？
很疑惑，是否需要时态的一致性？

不是呀这么说的呀。你看，这里的从句是一个比较结构，比较过去和现在，现在的谓语不是用了disputes现在时么？然后比较过去spent on health care last year当然用过去式啊~~~然后主句是doctor根据比较得出的评价，是现在时。
