我觉得in reference to和referring to没什么大的区别，顶多referring to更能表现主语smokers的动作关系。

而with reference to 则跟上两个大不相同。with表伴随，所以就会歧异为：在smoker表达焦虑 的时候会有一种reference的现象伴随产生。并不强调smoker和reference间的施动和受动关系。
