Need help -- HBS & Wharton Recommandation Form

Does anyone know what questions HBS and Wharton will ask recommanders to answer for Fall 2009 applicants?

I found the following essay questions for HBS but was wonderring what the rest of the form look like:

Recommender Questions for the Class of 2011

Recommendations must be completed online. The form includes the following three essay response questions, along with other types of questions.

§         Please comment on the context of your interaction with the applicant. How long have you known the applicant and in what connection? If applicable, briefly describe the applicant's role in your organization. (250-word limit)

§         Please describe the most important piece of constructive feedback you have given the applicant. Please detail the circumstances and the applicant's response. (250-word limit)

§         Please make additional statements about the applicant's performance, potential, or personal qualities you believe would be helpful to the MBA Admissions Board. (250-word limit)

For Wharton, I only found the form for Ph.D applicants (available for download on the website). Does anyone know what questions are contained in the MBA recoomender form?

The recommenders for your HBS application will be asked to fill out basic contact information and rate the candidate on a number of criteria. 

For Wharton, you should be able to see the recommendation forms.

To submit paper-based recommendations, download the Recommendation Form from the online application system, print one copy for each recommender, and follow the directions provided on the form.

Wharton Recommendation Questions:

Format Requirement:

• Ask them to answer all of the questions in a single document.

• Single space their answers.

• Separate their answers by writing each question at the top of each response.

1) How long you have known the applicant and describe your relationship to the applicant?

2) How would you rank this applicant relative to his/her peer group? Please describe the peer group to which you are comparing this applicant (e.g., employees with comparable work experience and level of responsibility).

3) How would you describe the applicant's personal integrity?

4) What observations do you have concerning the applicant's leadership abilities, team and/or group skills?

5) How would you describe the applicant's sense of humor (seriously)?

6) What are the applicant's most salient strengths?

7) What are the applicant's weaknesses? What efforts has the applicant made to address these areas?

8) What do you see this candidate doing and achieving in 10 years?

9) Do you have any additional comments that you think would assist the Admissions Committee in making its decision? If yes, please elaborate.

I find it very troublesome to ask my ex-managers to respond to such Q&A style recommendation letter as Wharton's.  This means they cannot use one general version of the letter for all the schools I'm applying to.  I feel it's asking too much from them to write a seperate letter for each of the 5 schools I'm applying to.

How do you guys deal with it?

Just had a chance to check this -- have been on project all week long... it has been quite tough to run multiple threads at the same time.

Recommendations

The Admissions Committee requires two recommendations. You have two options for submitting these letters: electronic and paper. Recommenders with Web access and an e-mail address should submit recommendations electronically. After you have contacted them about providing you a recommendation, complete the recommendations request form on the Recommendations page, and check "Please e-mail this person a recommendation request" beneath each recommender you wish us to contact. They will be sent e-mail messages with instructions and a Web address they should go to in order to complete your recommendation. You will then be notified by e-mail when we receive each recommendation.
