you may put "please refer to the attached recommendation letter" in the form, and attach the form. But you'd better answer each school's questions in the recommendation letter. Using a general recommendation letter is not very good.
usually you are allowed to upload a separate recommendation letter. If not, you have to break the letter into answers for different questions, or mail your letter.
