CBS 官网上说了 if you have been working full time for at least 6 months, the recommendation should be ideally from your current supervisor. 如果现在的上级不能提供推荐信 please submit a statement of explanation in the Employment section for your application.

我写了一个简短的解释。
只是提交完申请才想到5年前的supervisor会不会也太久了点。
