求问各位大牛

小弟刚收到UTD的信件，要求注射什么脑膜炎疫苗还有证明啥的，
是不是所有申请者今年都有这个要求啊？
怎么看也不像是AD的样子

信大概是这么写的：
Please be advised that effective January 2012, all new students are required to show evidence of being immunized against bacterial meningitis 10 days before the first day of class for the term of registration (Texas Education Code 51.9192, Subchapter Z).

To prevent delays in your registration, you must submit documentation of your immunization from an appropriate medical professional prior to registration as well as the Meningococcal Meningitis Vaccine Requirement Form. Documentation must be submitted to the Office of the Registrar or faxed to 972-883-6335.

You will be exempt if you:
--Are 30 years of age or older.
--Enrolled in a program that is completely online.
--Submit an affidavit or a certificate signed by a physician who is duly registered and licensed to practice in the United States stating, in the physician's opinion, the vaccination required would be injurious to your health and well-being.
--Submit a signed Texas Department of State Health Services Affidavit stating that you decline the vaccination for bacterial meningitis for reasons of conscience, including a religious belief.
