有申请普渡大学的吗？过来看下帮帮忙

本人打算申请普渡大学，可是对transcripts的要求我很迷惑，

One official transcript from each of the colleges and universities you have attended as a registered student, regardless of the number of courses taken, is required. Please use a Transcript Request Form (can be downloaded by clicking on the link) and ask the registrar of the institution you attended to enclose both your official transcript and the request form. The registrar should seal the envelope, sign across the seal, and return to you. The sealed envelope containing your transcript should be enclosed with your Supplemental Material. Additional request forms, if needed, can be downloaded from our web page.

If your native language is not English, you must send both an official transcript in the language of instruction and a certified official English translation. If your degree is not shown as having been awarded directly on the transcript, then you must also provide a certified official copy of your diploma in the language of instruction and a certified official English translation.

这是不是说除了提供中英文成绩单，request form 之外，我还要提供学历学位证明阿，有人知道这怎么办理吗？
