bloomingtree, you are right, you cannot pay the application fee of LBS online. So, as you already said, download, print and complete the Application Fee Payment Form. If you wish to pay with credit card, fill out those blanks corresponding to credit card. If you want to pay with cheque, I think you need to mail the cheque together with the other paper-based materials (not quite sure for this, maybe you can also mail the cheque at the bank)
If you can secure a fee waiver status, then choose none of the paying method in the payment form. But you should include a note with your application to let lbsknow that some current student or alumi have recommended you to the school (in addition to the two recommendation letters)and that this is why you have not included your fee.
bloomingtree, as I said, I am not that sure about how to pay with cheque. If it is possible, I think it is more convenient to use credit card. I think the best way is to ask the admissions committee and the sooner the better!
As for fee waiver, as far as I know, if you can ask a current student write a reference letter for you, then you can be granted a fee waiver. This has not been descibed on the net but is definitely true.
