这题答案是A，结果我做题的时候看到A就把它给排除了，本人一直认为 by failing ..... 修饰有歧义，完全可以理解为 accused the bank .... by .... 这样的话就是A选项就有错误修饰 accused 的嫌疑了呀为什么A是对的呢？

我查了一下，accuse常与of连用，accuse sb. of doing sth，所以后面的by并无修饰accuse的嫌疑
accuse【语法用法】
accuse的宾语一定要是被控的人的词，宾语后接of和指所控的罪或过失的词。
They accused him of murder.
他们控告他谋杀。
accuse后不可接that从句，如不可说She was accused that ...；
accuse sb of sth/of doing sth中of后强调“指责，指控的内容”。
They accused Tony of cheating.
他们指责托尼作弊。

想多问一句，这题我做的时候选了正确答案A，但是回过头细想又说不出什么能说服自己的理由，请各位大神支招
