答案的思路是根据单双数可以去掉bde,剩下a和c,然后因为ranks as 简单且优于has the rank，所以选a。
本身题目搞清楚了，就想问问only surpassed 和surpassed only有没有区别，我感觉没有区别。

only surpassed中only修饰surpassed，错，这里强调的是只有心脏病和癌症，所以only应该放在surpassed后面去修饰by...这个介词短语。

2) 词在句中的位置
 要关注一些重点词，all, only；位置不同句子意思也不一样。
EG: ONLY the council votes on Thursdays.
The council votes ONLY on Thursdays.

这样是不对的，only在这是修饰整个介词词组by heart disease and cancer，所以要放在By前。
