加拿大WLU的coop MBA不写PS吗？

加拿大WLU的coop MBA不写PS吗？因为没看到申请材料里面提到了这个，不过在PERSONAL INFORMATION FORM - MBA中part4 ADDITIONAL INFORMATION里倒是要求写出一下几点：

1. List your significant business or professional achievements, awards, distinctions and honours.
2. Describe your extra-curricular activities while attending university or college.
3. List community and professional activities in which you have participated.
4. Describe your career aspirations and why you have decided to enter Laurier’s MBA program at this time.
5. The Admissions Committee is not only attempting to identify candidates with strong academic potential, but also those
with strong potential for future success in a professional environment. What additional information concerning such
areas as leadership ability, entrepreneurial interests or interpersonal skills can you supply in support of your candidacy?

这个是糅在一篇里写，还是分条表述呀？

呼唤楼主，或者哪个好心人看到能给个回复呗，，楼主申请的checklist 第七条是要求提交一个英文版的课程论文嘛。。我已经问过小米，焦急等待回复中。。
