关于Finacial Statement 中的SPONSOR LETTER

有些学校要求填写的FINANCIAL STATEMENT 中要求提供notarized statement  of individual(Parents) sponsors. 学校提供的样本上看要填写一个NOTARY SIGNATUER. 是不是就填个律师的名字就可以了,还是要去公证? 原话如下:

On this 5th day of April, 2006 before me personally appeared (parent's name), to me known to be the individual whose signature appears above and acknowledge that he executed the same.

另外,签证时要带上公证的亲子证明,不知道要怎么办?
