不觉得 E 的 it 会有歧义。

从意思上判断it指代了那个宣言，但是E里面有两个单数名词，宣言和大会，而且it离大会更近一些，所以不太好判断吧

'A' and 'B' are awkward and wordy constructions and are therefore wrong.
'Declaration of Sentiments' and 'inclusion of a call for ...' happened at almost the same time. Hence past perfect 'had been included' in 'C' is incorrect tense.
'D' has no main clause and is therefore wrong.
In 'E' 'it' refers to 'declaration' which is subject of the previous clause. The construction is clear and concise.

至于E中句末it指代的问题，相比C要清晰很多，E是when连接的主从句，句子结构是:
